The online form (below) must be entered in one sitting, therefore….

1) Compile your Program Submission Worksheet using this Word document.
2) If there are co-presenters, they will need to complete a Co-Presenter Form.
3) At the end of the online submission, you will be required to attach all Co-Presenter Forms.
4) Once your Program Submission Worksheet is complete, cut and paste the information into the online form (below).
5) Required fields are indicated by an asterisk “ ∗ ”.
