Position Summary
The Center for Health and Wellness supports the learning environment of Middlebury’s vibrant and diverse academic community by providing readily accessible and confidential health care to students within an inclusive, collaborative, and integrated treatment framework. Reporting to the Executive Coordinator, the Director of Integrated Care is responsible for developing, implementing, and evaluating a comprehensive continuum of evidence-based case management strategies and transition-related services which promote resilience, persistence, safety, and retention. The Director maintains high student contact and an ongoing caseload to address the clinical needs of students in areas such as mental or physical health, finances, discipline, social injustices, and adjustment to college that may affect their academic performance or other aspects of their Middlebury experience – using a variety of interventions, referrals, and follow up services. Responsibilities include student contact via regular meetings, triage, case management, and engaging and consulting with Center for Health and Wellness staff, Dean of Students staff, faculty, administration, and outside treatment providers to ensure optimal student care. Their work should contribute to a healthy campus community that actively encourages and supports students’ ability to care for themselves, access College and community-based resources, and effectively navigate leaves of absences to seek higher levels of care when indicated. The Director will engage in relationship building, data disaggregation, and clinical interventions to address connections between culture, identiti(es), and social justice as determinants of health for groups of students who have been historically underrepresented or marginalized including but not limited to students of color, first generation students, and queer students. This is a 12-month, exempt, benefits eligible position.
Essential Functions
Offer is contingent upon successful completion of a criminal background check.
Provide case management and related clinical support services for students on a wide variety of adjustment, distress, and trauma issues. This includes, but is not limited to, assessing issues of concern for students, providing immediate assistance, making long-term referrals, and conducting follow-up to determine the success of and/or the need for additional referrals or interventions.
Assist students in accessing services at the College and in the community by collaborating with Center for Health and Wellness staff, Dean of Students staff, the Center for Teaching, Learning, and Research, the Anderson Freeman Resource Center, and other College departments and off-campus treatment providers and resources.
Ensure students receive appropriate interventions, referrals, follow-up services and monitoring in order to maximize student success and minimize community members’ distress.
Make on-going, independent decisions around risk management, referrals, and confidentiality related to daily operations and crisis management support.
Facilitate the Center for Health and Wellness clinical case consultation meeting to review students of concern and cross-reference students identified by Dean of Students staff.
Co-chair the Division of Student Affairs’ Care Team with the Assistant Director of Student Success who provides non-clinical case management for students with a focus on students’ academic, social, and residential community-based lives.
Create and provide summary reports for weekly Center for Health and Wellness Case Management Meetings and, when a release of information is obtained, Care Team.
Meet directly with and develop educational and training plans in collaboration with on-and off-campus partners, including the Assistant Director of Student Success, to create proactive and preventive approaches to students of concern.
Request, organize, and summarize healthcare records for students seeking referrals from Counseling to Health Services for mental health medication initiation and/or management.
Consult with parents, faculty, college administrators, and Student Affairs staff as appropriate.
Ensure clinical services and record-keeping are in ethical and regulatory compliance.
Create and deliver training materials and workshops for University departments and programs to help staff and faculty recognize signs of distress and address the needs of students in crisis.
Build and maintain effective liaison relationships with off-campus mental health providers, private practitioners, hospitals and inpatient units, and other community partners.
Establish and maintain a comprehensive database for healthcare providers in Addison County Vermont, and New England and assist students in finding health care near their homes as needed.
Develop and implement effective systems for follow-up with students who have been referred to community resources to ensure they are successfully referred.
Facilitate substance use, psychiatric, and other assessments, as needed, including communicating with community providers, obtaining assessment documentation, and sharing recommendations with appropriate College staff and offices to coordinate continued care.
Facilitate psychiatric hospitalizations, as needed, including communicating with hospital case managers or intake specialists, obtaining discharge summaries, and communicating with providers.
Provide follow up based on disposition following discharge (e.g., return to campus, leave of absence) including tracking cases to ensure student compliance with leave/return recommendations, and referral to appropriate on-and off-campus support.
Serve as the point of contact for Medical Return Forms including supporting students in their completion, reviewing and tracking receipt, contact with off-campus treatment providers, and communicating recommendations for care continuity to Student Life deans and/or the Assistant Director for Student Success to ensure adequate on- and off-campus support.
General Responsibilities
Participate in administrator and/or clinical provider on-call rotations as directed.
Represent the Center for Health and Wellness on committees and task forces as appointed/assigned on campus and in Addison County
Maintain and develop skills via participation in continuing education opportunities and membership in the American College Health Association (ACHA) and attendance at the ACHA New England College Health Association (NECHA) affiliate annual meetings
Develop and maintain professional relationships with counterparts at peer institutions including engagement in peer review processes
Other duties as assigned
Some evening and weekend hours are required
Qualifications
Education
Master’s degree required in counseling, psychology, social work, education, or related clinical field with current, or willingness to obtain, Vermont licensure. Preferred candidates will have training, certification, and/or formal education in higher education, adolescent or young adult psychology, systems management, case management, and community mental health.
Knowledge, Skills and Abilities
Candidates should possess strong leadership, organizational, clinical, and communication skills; strong intervention skills one-on-one, in small groups, and large groups; strong technology skills (presentation software, word processing, social media); an ability to meet deadlines; handle multiple, complex, and abstract tasks; capacity for detail-oriented work; an ability to work independently as well as part of a team; an understanding of student development and behavior change theories; advanced training and skills in clinical health and intervention delivery; demonstrated ability to identify, make, and track referrals to the network of campus and community services for students; demonstrated use of multicultural approaches to clinical care; commitment to addressing intersections of multiple identities and their related health outcomes; proven record of successful work with college students, faculty, staff and community members from diverse cultural and educational backgrounds including socio-economic, racial/ethnic, international, first generation college students, and LGBTQ populations; and commitment to public health and social justice change models.
Additionally, candidates should possess advocacy, programmatic, and clinical skills related to case management/coordination and crisis intervention.
Experience
At least five years of professional experience in college mental health counseling, case management, or student success; social work; student affairs administration; community health/mental health; patient advocacy; and/or training and technical experience in aforementioned areas desired. Experience working in higher education/student affairs preferred. Unpaid or volunteer experience in these fields may be applied to the five year professional expectation but may not replace the professional experience entirely; candidates who wish to apply volunteer experience should demonstrate how their experience is equivalent to professional experience.
Ideal candidates would also bring experience in community needs assessments, program planning, community-based evaluation methodologies (such as Empowerment Evaluation), and assessment; 1:1 clinical work, building, maintaining, and facilitating small group experiences and community-wide campaigns or programs; developing, facilitating, and coordinating professional development opportunities for adult learners; and developing and editing educational and communication tools (web, print, social media).
Candidates with training experience and/or certification in the following areas/strategies are strongly preferred: behavioral intervention teams/Care Teams, crisis management, safety assessments, care planning, care management, motivational interviewing, and brief negotiated interviews. Candidates should identify trainings and proficiencies in other similar or related areas in application materials.
Physical Demands
Regularly operates a computer and other office productivity machinery. Must be able to remain in a stationary position 50% of the time. Will need to regularly move about inside the office and will require regular cross-campus travel to access resources and personnel in other buildings. Will require periods of standing (or other postures) to deliver programming and/or facilitate groups. May require limited and infrequent in-state and out-of-state trips for trainings, professional meetings, and professional development.
